XQ4004 Quality Tools and Quality Processes: Six Sigma Infographic – Sample Assessment
XQ4004 Assessment Instructions Summary
The XQ4004 Performance Task is a two-part assessment. Part 1 requires an infographic explaining one quality improvement model or process—Six Sigma, FADE, or PDSA—and showing how the selected approach can be applied in the learner’s practice setting. Part 2 requires completion of a fishbone template to show how quality-improvement tools can help solve a practice problem.
The assessment should use scholarly writing conventions and appropriate APA citations and references. The required submission consists of an infographic and a completed fishbone template.

Six Sigma
What Is Six Sigma?
- Six Sigma is an approach that uses data to reduce errors and help eliminate variability in process work.
- Developed by Motorola, adopted widely in healthcare
- Seeks to achieve "near perfection" — only 3.4 defects per million opportunities
- Uses the DMAIC framework for continuous improvement (Adeodu et al., 2023).
The DMAIC Framework (Core of Six Sigma)
Application Example: Reducing Patient Wait Times in Emergency Department
Problem: Long wait times affecting patient satisfaction and outcomes.
Outcome
- Wait times decreased by 30% within 3 months
- Patient satisfaction scores improved by 18%
How Six Sigma Helps:
- Define: Long wait times >90 minutes on average
- Measure: Collected timestamps from patient arrival to triage and doctor consult (Qayyum et al., 2021).
- Analyze: Used a Fishbone Diagram to reveal root causes like outdated triage protocols, poor communication, and limited staff during peak hours (Kam et al., 2021).
- Improve: Introduced an electronic triage system and adjusted nurse staffing levels (Daniyan et al., 2022).
- Control: Monthly audits and real-time dashboards to sustain reduced wait times
DMAIC Framework Table
| Phase | Description |
|---|---|
| D – Define | Identify the problem and scope of improvement. |
| M – Measure | Collect data to understand the current performance. |
| A – Analyze | Examine root causes using tools like Fishbone or Pareto charts (Rathi et al., 2022). |
| I – Improve | Implement solutions to remove root causes. |
| C – Control | Monitor changes and sustain the gains through standardization. |
