In my organization, I would implement change by first identifying a quality or patient safety issue through data collection, staff feedback, and incident reports. For example, if medication administration errors were increasing, I would review error trends, identify contributing factors, and present the findings to nursing leadership and staff. Using Lewin’s Change Theory, I would begin with the unfreezing stage by creating awareness of the problem and explaining why change is necessary. Next, during the change stage, I would introduce evidence-based interventions such as barcode medication administration, staff education, and standardized medication administration procedures. Finally, during the refreezing stage, I would monitor outcomes, provide ongoing education, and incorporate the new practices into organizational policies to ensure long-term sustainability (Burnes, 2020).
Collaboration and communication are essential for facilitating successful change. Effective communication helps ensure that all stakeholders understand the purpose of the change, expected outcomes, and their role in the process. Open communication also allows staff to express concerns, ask questions, and provide feedback, which can increase buy-in and reduce resistance. Collaboration promotes teamwork among nurses, physicians, pharmacists, administrators, and other healthcare professionals. When team members work together toward a common goal, they are more likely to support the change and contribute ideas for improvement. According to Spath and Devane (2023), quality improvement efforts are most successful when healthcare professionals engage in interdisciplinary collaboration and maintain a shared commitment to patient safety and quality outcomes.
